Free Fall Protection Inspection Checklist

Falls are the #1 killer in construction, and fall protection must be inspected before every use. This free checklist covers the personal fall arrest system (harness, lanyard, connectors, anchor) plus guardrails and openings. Print it, or snap a photo and let the app flag what's missing.

Harness (full-body)

  • ☐ Webbing: no cuts, frays, burns, or chemical damage
  • ☐ Stitching intact — no pulled or broken threads
  • ☐ D-rings: not bent, cracked, or corroded; move freely
  • ☐ Buckles and grommets: function, no distortion
  • ☐ Labels legible; within service date

Lanyard / SRL (self-retracting lifeline)

  • ☐ Shock pack: not deployed or torn
  • ☐ Snap hooks / carabiners: self-lock and close fully
  • ☐ SRL retracts and locks when pulled sharply
  • ☐ Cable/web: no kinks, cuts, or corrosion

Anchorage

  • ☐ Rated for 5,000 lb per worker (or engineered system)
  • ☐ Located to limit free fall to 6 ft or less
  • ☐ Positioned to prevent swing-fall hazards

Guardrails, covers & openings

  • ☐ Top rail ~42 in; midrail in place; can withstand 200 lb
  • ☐ Floor holes covered, secured, and marked
  • ☐ Leading edges protected; warning lines where used
