Free Fall Protection Inspection Checklist

Harness (full-body)
- ☐ Webbing: no cuts, frays, burns, or chemical damage
- ☐ Stitching intact — no pulled or broken threads
- ☐ D-rings: not bent, cracked, or corroded; move freely
- ☐ Buckles and grommets: function, no distortion
- ☐ Labels legible; within service date
Lanyard / SRL (self-retracting lifeline)
- ☐ Shock pack: not deployed or torn
- ☐ Snap hooks / carabiners: self-lock and close fully
- ☐ SRL retracts and locks when pulled sharply
- ☐ Cable/web: no kinks, cuts, or corrosion
Anchorage
- ☐ Rated for 5,000 lb per worker (or engineered system)
- ☐ Located to limit free fall to 6 ft or less
- ☐ Positioned to prevent swing-fall hazards
Guardrails, covers & openings
- ☐ Top rail ~42 in; midrail in place; can withstand 200 lb
- ☐ Floor holes covered, secured, and marked
- ☐ Leading edges protected; warning lines where used
