Free Job Hazard Analysis (JHA) Template

Use this free job hazard analysis template (also called a JSA) before any non-routine task. Break the job into steps, name the hazard in each step, and assign a control using the hierarchy of controls. Print it for the truck binder, or run the same check from your phone in seconds.

For each job step, record three things

Add a row per step — usually 4 to 10 steps for a task.

  • STEP: what gets done, in order
  • HAZARD: what could go wrong in that step (be specific — 'fall from ladder on uneven ground', not just 'falls')
  • CONTROL: how you eliminate or reduce it (strongest option first)

Hierarchy of controls — pick the strongest that works

PPE is the LAST resort, not the first answer.

  • 1. Eliminate — do the work a different way
  • 2. Substitute — a safer method or material
  • 3. Engineering — guardrails, trench boxes, machine guards
  • 4. Administrative — procedures, training, signage, rotation
  • 5. PPE — hard hat, harness, eye/hand protection
